The State of Hawaii Med-QUEST Division (MQD) provides eligible low-income adults and children access to health and medical coverage through managed care plans. The QUEST program is designed to provide Quality care, Universal access, Efficient utilization, Stabilizing costs, and to Transform the way health care is provided to recipients.
Services: provides partial payment for qualified applicants for mortuary expenses (up to $400) and burial expenses (up to $400). Claim will be denied if not submitted within sixty days from the date of death. Hawaii DHS Form 1163 is to be completed and returned to a DHS office.
Eligibility for the death payments program shall be limited to:
(1) Medical or financial assistance recipients who were residents of the State at the time of death; or
(2) Unclaimed bodies.
