Inter-disciplinary team coordinates a wide range of services for the frail or vulnerable elderly. Wide variety of
services include case management, nursing, personal care, consultation, long-term care assessments, caregiver support, and followup.
Eligibility: 60+, frail & vulnerable.
Accommodations for handicapped: yes.
Info/certificates needed when applying: completed referral form from agencies or a call from a client or family. Home visit by registered nurse for intake.
