Description:
Services offered: palliative care (pain management)provided by a multi-disciplinary team of professionals & volunteers for terminally ill in patient's home. Care is under direction of MD. Intermittent home visits by: RN, home health aides, social worker, physical therapists, occupational therapists, speech therapists, pastoral services, volunteers, and medical equipment suppliers.
Eligibility: patient must be certified by a physician that he/she has a terminal illness with a life expectancy of 6 months or less. Must have a caregiver at place of residence.
Accommodations for handicapped: main office is for staff only. Care of patient occurs at patient's residence.
