Description:
Provides case management and counseling services for unpaid family caregivers and their care receivers. Services include: Assessment of caregivers and care receivers needs; Arranging for needed community services to help preserve family caregiving situation; Education and counseling about caregiving. Bi-monthly support groups are held in Ewa and Honolulu. Caregiver Training is held in both a group and individual setting. Financial assistance for specific needs such as assistive technologies and incontinence supplies. Eligibility: There must be an unpaid caregiver involved with the client. Supplemental services are only for frail care receivers that need help with least 2 activities of daily living.
