Description:
Services provided: Specializes in geriatric case management. Provides case management: in home assessment of safety and personal needs; apporpriate in-home care and monitoring; assistance with medical appointments, purchases; assistance with relocation to qualified care home, assisted living facility, nursing facility. Provides in-home care: personal care attendents are supervised by experienced care managers. Provides screening of care attendents. Hours: Monday - Friday, 7:30 am - 5:30 pm. Target group: 60+. Eligibility: primarily for 60+, but will serve others, especially persons with disabilities. Application process: call for information and appointment. Initial interview done at client's home.
