Description:
Services provided: adult day care facility licensed by the State of Hawaii. Program includes lunch and snacks. Hours: Monday - Friday, 6:45 am - 5:00 pm. Eligibility: 1) 55+; 2) living alone or in need of supervision and care while family/caregivers are at work; 3) in need of social and mental stimulation to prevent institutionalization.
Service Rate: Fulltime: $1150/month Parttime: $70 per day;
$25 non-refundable intake fee. $10 late pickup fee, 5:00-5:15pm; $20, 5:15-5:30pm; $35, after 5:30-5:45pm; $1/minute each minute after 5:45pm. Licensed.
Eligability: anyone 55 years and older. Must be able to ambulate with one-person assist (with or without an assistive device). Must be able to feed themselves. Cannot be combative or disruptive. Is not prone to wandering. Schedule appointment for assessment. Complete a physical examination. Complete a 2 Step TB Test.
