Description:
Description of Services: Adult Day Care licensed for 6 clients at a time. Location is in a home-like enviornment and clients receive lunch and 2 snacks. Fees: $66/day which include 2 snacks and lunch. Deposit of $180, wich is refundable if given 2 weeks notice to terminate services or client no longer meets minimum eligibility requierments, e.g. bedriden, combative.
Eligibilty: Complete application package which includes a physical exam by a physician, 2 step TB test, and passing an initial assessment of behavior and physical capabilities.
Hours: Monday - Friday from 7:30 am to 5:00 pm.
